Evolution of peripheral vestibular spontaneous nystagmus

Este tipo de nistagmo é um sinal cardinal indicando que existe um desequilíbrio estático entre os inputs de repouso dos dois sistemas vestibulares periféricos.
This type of nystagmus is a cardinal sign indicating that a static imbalance exists between the resting outputs of the two peripheral vestibular systems. (1)

Os transtornos periféricos agudos do labirinto e/ou da porção vestibular do VIIIº nervo geralmente resultam em um nistagmo horizontal-rotatório fixo em direção, que aumenta a amplitude quando a visão é negada.
Acute peripheral disorders of the labyrinth and vestibular portion of the VIIIth nerve commonly result in a direction-fixed horizontal-rotary nystagmus that increases in amplitude when vision is denied. (1)

02/08/2019: x,masc., 48 anos: Há 48 horas iniciou com tonteira rotatória com náuseas e vômitos. Não apresentou sintomas auditivos. Doze horas depois foi atendido na emergência com elevação da pressão arterial (200x110mmHg).

Para deficiências do sistema vestibular periférico, o nistagmo espontâneo deve aumentar a velocidade quando o paciente desvia o olhar na direção da fase rápida, diminui a velocidade quando o paciente olha na direção da fase lenta. Essas características seguem o que foi chamado de lei de Alexander (Alexander, 1912).
For peripheral vestibular system impairments, the spontaneous nystagmus should increase in velocity when the patient deviates gaze in the direction of the fast phase, become smaller in velocity when the patient stares in the direction of the slow phase. Those characteristics follow what is has been called Alexander`s law (Alexander, 1912). (1)

O nistagmo espontâneo devido a uma assimetria nos inputs entre os dois labirintos gera a ilusão de um movimento rotatório na ausência de qualquer movimento da cabeça. Especificamente, os pacientes descrevem que sentem que estão girando em direção ao lado sadio. O nistagmo espontâneo na perda aguda da função vestibular periférica é principalmente horizontal com um componente torcional.
Spontaneous nystagmus due to an asymmetry in the neural resting rate between the two end organs generates the illusion of rotary motion in the absence of any head movement. Specifically, patients describe that they feel as though they are rotating toward the intact side. Spontaneous nystagmus to an acute loss of peripheral vestibular function is primarily horizontal with a torsional component. (1)

Head impulse test: O dano no sistema vestibular periférico esquerdo resultará em uma sacada de re-fixação para a direita na virada da cabeça para a esquerda, e na manutenção da fixação normal, através de um reflexo ocular vestibular normal, com a virada da cabeça para a direita.
Head impulse test: Damage to the left peripheral vestibular system will result in a catch-up saccade to the right on left head turn, and the maintenance of normal fixation through a normal vestibular ocular reflex with right head turn.

O paciente foi medicado com flunarizina 1 vez ao dia e betaistina 24mg duas vezes ao dia, e foi solicitado exame audiométrico e ressonância magnética das mastóides.
The patient was treated with flunarizine once daily and 24 mg betahistine twice daily, and audiometric examination and mastoid magnetic resonance imaging were requested.

09/08/2019 – Nono dia da crise vertiginosa: O paciente refere melhora considerável com a medicação prescrita. Relata desequilíbrio durante a marcha e movimentos da cabeça. Exame audiométrico e estudo com ressonância dos ouvidos foram normais.
08/09/2019 – Ninth day of dizzying crisis: The patient reports considerable improvement with the prescribed medication. Reports imbalance during gait and head movements. Audiometric examination and study with resonance were normal.

Observem no video acima que agora, no nono dia da crise vertiginosa, o nistagmo com os olhos abertos aparece somente com o olhar para a direita, em virtude dos mecanismos de compensação vestibular.
Notice in the video above that now, on the ninth day of the dizzying seizure, nystagmus with eyes open appears only when the patient looks to the right side, due to vestibular compensation mechanisms.

Porém, quando eu peço para o paciente fechar os olhos (ausência de fixação visual), consigo observar através das pálpebras o nistagmo espontâneo original. Observem no video abaixo:

A capacidade de identificar nistagmo espontâneo após comprometimento unilateral do sistema vestibular é maior nos primeiros 3 a 7 dias. A compensação vestibular usa um mecanismo adaptativo mediado centralmente para restaurar a atividade neural no lado comprometido, o que resulta em uma redução na amplitude do nistagmo espontâneo e na cessação da percepção de vertigem do paciente.
The ability to identify spontaneous nystagmus following a unilateral vestibular system impairment is greatest within the first 3 to 7 days. Vestibular compensation uses a centrally mediated adaptive mechanism to restore neural activity to the impaired side, which results in a reduction in the amplitude of the spontaneous nystagmus and a cessation of the patient’s perception of vertigo. (1)

A conduta foi suspender a flunarizina, manter a betaistina e encaminhar o paciente para iniciar exercícios de reabilitação vestibular.
The conduct was to discontinue flunarizine, maintain betahistine and refer the patient to begin vestibular rehabilitation exercises.

20/08/2019: Vigésimo dia da crise vertiginosa: Não se observa mais o nistagmo espontâneo no exame físico não instrumentalizado. O paciente refere que está bem melhor e o Head impulse test (HIT) mostra a deficiência do reflexo vestíbulo oculomotor do lado esquerdo (HIT positivo com o giro para o lado esquerdo – Vídeo abaixo):
08/20/2019: Twentieth day of the dizzying crisis: Spontaneous nystagmus is no longer observed on non-instrumented physical examination. The patient reports that he is much better and the Head impulse test (HIT) shows the left side oculomotor vestibular reflex deficiency (positive HIT with the left side rotation – Video below):

Conduta: Paciente fazendo programa de reabilitação vestibular e usando 48mg/dia de betaistina.

26/08/2019 – Paciente sente-se assintomático, apesar de orientado do contrário já andou novamente de motocicleta, e refere que muito raramente tem um desequilíbrio rápido ao girar o corpo nas atividades diárias. Em uso de 48mg/dia de betaistina e no programa de reabilitação vestibular.

VHIT: Presença de sacadas corretivas (catch-up saccades) de tipo overt e covert com giro da cabeça para o lado esquerdo no plano horizontal e sacadas do tipo overt no plano do canal anterior esquerdo.
Velocidade dos olhos não acompanha a velocidade da cabeça no giro da cabeça para o lado esquerdo no plano horizontal e no plano do canal anterior esquerdo.
Redução de ganho do reflexo vestíbulo-oculomotor (RVO) para os canais lateral esquerdo (média de ganho de 0,24) e canal anterior esquerdo (média de ganho de 0,55).

VHIT: Presence of overt and covert catch-up saccades with head rotation to the left in the horizontal plane and overt saccades in the left anterior canal plane.
Eye speed does not keep up with the speed of the head when turning the head to the left in the horizontal plane and the plane of the left anterior canal.
Vestibulo-oculomotor reflex (VOR) gain reduction for the left lateral channel (mean gain 0.24) and left anterior channel (mean gain 0.55).

Os achados são compatíveis com hipofunção dos canais anterior e lateral esquerdo (divisão superior do nervo vestibular).
The findings are compatible with hypofunction of the left anterior and lateral canals (superior division of the vestibular nerve).
